Dine In Plugin Update

Hi, 

This plugin is great however a few little changes would make it so much better. 

1) We use the table layout, when you select takeway, it then asks for a table number, Ideally this would bypass the table layout and give it a auto generated number for call out. 

2) In the UK we have different VAT rules for takeout/eat in for our café. Allowing this would allow easier use instead of manually changing it.
